Once you download Wakeful, you will be asked to create an account or login.  If it's your first time create an account.  Afterward you login or create an account, you'll be brought to a screen where you can change your location, set your alarm and change voice settings.

Wakeful will act as a trial for 7 days. Once your trial expires, you'll be asked to purchase. Then just enter in your registration code.
